A Note To School excuse notepad form for parents to teachers with 40+ unused pages left (41 to be exact if the count was correct). Yellow gold notes are about 8.5 x 3.75 inches. These notepads were sold by a local PTA to raise funds. The top reads A NOTE TO SCHOOL with lines below to fill in designated for the date, teacher name, parent signature, student name then a list of options to check with space for details including reasons for lateness, illness, pickup changes, trip request & other. There is one small scribble on the back of the pad itself.
